Clinicians most often consider AIP for people with a diagnosed autoimmune condition who have not achieved adequate symptom control through standard medical treatment alone, generally recommending it as a complement to, rather than a replacement for, ongoing medical management under the close supervision of their treating physician throughout the process.
The personalized nature of the reintroduction phase offers a genuine structural advantage over less individualized elimination diets, since it is specifically designed to identify each person's own particular triggers rather than assuming a single, universal food list applies equally well to every patient regardless of their specific condition, history, or genetic background.
Drawbacks, Health Risks, and Criticism
The leaky gut theory underlying AIP's entire design is more contested than proponents sometimes present it, particularly when used as a single, unifying explanation across a wide range of unrelated autoimmune conditions rather than the narrower, condition specific role the current evidence more clearly supports for a smaller subset of well studied diseases.[4]
Eliminating grains, legumes, dairy, nightshades, eggs, nuts, and seeds simultaneously removes several major food groups at once, a genuinely restrictive combination that raises real questions about calcium, vitamin D, fiber, and other micronutrient adequacy if not carefully planned, making medical and dietitian oversight important rather than optional for anyone attempting the full elimination phase as originally written and intended by its creators.
The current evidence base remains small and preliminary: published pilot studies typically involve fifteen to twenty participants without control groups, a scale that supports genuine optimism but falls well short of the larger, controlled trials needed to confirm the diet's specific effect with real confidence rather than relying on early, promising signals from a handful of small studies alone.
