The Keto Diet, Explained
The ketogenic diet is a very high fat, moderate protein, very low carbohydrate eating pattern, typically around 55 to 75 percent of calories from fat, 20 to 35 percent from protein, and 5 to 10 percent from carbohydrate, designed to shift the body's main fuel source from glucose to ketones.
Its central principle is carbohydrate restriction severe enough, usually under 20 to 50 grams a day, to drop insulin levels far enough that the liver starts converting fat into ketone bodies for energy, a state called nutritional ketosis.
The main proven benefit is control of drug resistant epilepsy, its only universally accepted medical indication, while its broader use for weight loss and blood sugar control shows real short term benefit alongside a genuine, still debated long term question about its effect on cholesterol and cardiovascular risk.

Historical Background and Evolution of the Diet
Before insulin was discovered in the 1920s, doctors managing type 1 diabetes had little to offer beyond very low carbohydrate, near starvation diets meant to reduce sugar in the urine and blood, often at the cost of severe malnutrition, especially in children. Around the same time, physicians noticed that fasting seemed to reduce seizures in people with epilepsy, and in 1921 Russell Wilder, a physician at the Mayo Clinic, proposed that a diet very high in fat and very low in carbohydrate could mimic the metabolic effects of fasting without requiring a patient to stop eating entirely. He coined the term ketogenic diet for this approach and began using it to treat epilepsy that same year.[1]
The diet spread quickly through the 1920s, then faded almost as fast once new anticonvulsant medications arrived in the 1930s and insulin became the standard treatment for diabetes. It survived mainly at a small number of specialty epilepsy centers for the next several decades, treating children whose seizures medication could not control.[1]
Its return to wider attention in the 1990s had an unusually public trigger: a Hollywood producer whose son's severe epilepsy responded dramatically to the ketogenic diet after medications had failed, followed by national television coverage and a made for television film starring Meryl Streep. That renewed visibility funded new research centers and reintroduced the diet into mainstream pediatric neurology. From there, interest expanded well beyond epilepsy, into weight loss, type 2 diabetes, and, more recently, investigational use in conditions ranging from polycystic ovary syndrome to certain neurodegenerative diseases.[2]
Nutritional Principles and Macronutrient Breakdown
Clinical sources generally describe the ketogenic diet as roughly 55 to 60 percent of calories from fat, 30 to 35 percent from protein, and 5 to 10 percent from carbohydrate, though the exact ratio depends on which version is used. The strict traditional diet, still used for treatment resistant childhood epilepsy, runs closer to 90 percent fat, with fat weighed against combined protein and carbohydrate at a fixed ratio, most often four to one, and every meal measured precisely by a dietitian. A somewhat less rigid version called the modified Atkins diet uses roughly a one to one ratio instead, working out to about 60 to 70 percent fat, 25 to 30 percent protein, and 5 to 10 percent carbohydrate, without requiring food to be weighed, which makes it easier for teenagers and adults to sustain. A third version, often called the very low carbohydrate ketogenic diet and more common for weight loss or type 2 diabetes, skips fixed ratios entirely and instead caps carbohydrate at roughly 20 to 50 grams a day while letting fat make up 60 to 75 percent of calories and protein fill in the remainder.[1]

Physiological and Metabolic Mechanism
Dropping daily carbohydrate intake below roughly 20 to 50 grams pulls blood glucose and insulin down sharply. With insulin no longer signaling the body to store fat, fat cells release free fatty acids into the bloodstream, and the liver converts a portion of them into three ketone bodies: acetoacetate, beta-hydroxybutyrate, and acetone. Once glucose is scarce, the brain, heart, muscles, and kidneys start running on these ketones instead.[1]
Ketones are not simply a backup fuel; by one estimate, they generate somewhat more usable cellular energy per gram than glucose does, and they appear to lower the production of certain free radicals in the process, a possible piece of why the diet has drawn interest well beyond epilepsy and weight loss.[1]
This state, called nutritional ketosis, keeps blood pH normal and is distinct from diabetic ketoacidosis, a dangerous and unrelated condition in which uncontrolled diabetes floods the blood with both glucose and ketones at once. The distinction matters clinically: certain diabetes medications, particularly SGLT2 inhibitors, can combine with the carbohydrate restriction of a ketogenic diet to trigger a rare but serious variant called euglycemic diabetic ketoacidosis, in which ketoacidosis develops despite blood sugar that looks close to normal, which is why that medication class and this diet are not meant to be combined without close medical supervision.
Beyond fuel switching, ketone bodies appear to act as signaling molecules in their own right. Beta-hydroxybutyrate specifically has been shown to influence gene expression tied to inflammation, one proposed mechanism behind the diet's effect on seizures and its investigational interest in other neurological conditions, though researchers note the full picture is still being worked out.[1]
Proven Health and Medical Benefits
Epilepsy remains the ketogenic diet's only indication with true guideline level support, particularly drug resistant epilepsy in children, glucose transporter type 1 deficiency, and pyruvate dehydrogenase deficiency, where it can meaningfully reduce seizure frequency when medication alone has failed.[1]
For obesity and type 2 diabetes, the evidence is real but narrower than popular coverage sometimes suggests. A meta-analysis of randomized controlled trials in people with overweight, obesity, or type 2 diabetes found the ketogenic diet produced meaningful reductions in body weight, blood sugar, and triglycerides, alongside increases in protective HDL cholesterol, when compared with control diets. Much of the early weight loss reflects water lost as glycogen stores empty, but appetite reduction as ketosis sets in, plus preserved lean mass compared with some other restrictive diets, contribute to continued loss beyond those first couple of weeks.[3]
Some evidence also points to benefits in polycystic ovary syndrome, where carbohydrate restriction has improved insulin and androgen levels in small trials, and early, still investigational research is exploring the diet as a supportive approach in certain neurodegenerative conditions and some cancers, working through effects on tumor glucose metabolism, though clinical evidence in humans for those uses remains limited and it is not considered standard care outside a research setting.
Clinicians most often consider a ketogenic diet for children and adults with drug resistant epilepsy, and, with medical supervision, for adults with obesity, hypertriglyceridemia, or type 2 diabetes who have not responded well to other dietary approaches, particularly when improving insulin sensitivity and lowering triglycerides are the priority alongside weight loss itself.
Drawbacks, Health Risks, and Criticism
The short term adjustment period, commonly called keto flu, brings nausea, headache, fatigue, dizziness, and constipation as the body shifts fuel sources and loses fluid and electrolytes; roughly half of people starting the diet report some gastrointestinal discomfort in this window, which usually eases within days to a couple of weeks.[1]
The more debated long term risk involves cholesterol. Most people see triglycerides fall and HDL rise on keto, but LDL cholesterol response varies considerably, and a specific subgroup, lean, otherwise metabolically healthy people, can see dramatic LDL increases, sometimes several times their starting level, a pattern researchers now call the lean mass hyper-responder phenotype. What that means for long term cardiovascular risk in this specific group is still an open, actively studied question rather than a settled one.[4]
Other documented long term risks include kidney stones and hypercalciuria, reduced bone mineral density in some populations, particularly athletes and people with epilepsy on the diet for extended periods, and micronutrient deficiencies that generally call for supplementation. The diet is specifically not recommended during pregnancy, and it is contraindicated for people with a history of eating disorders, since its rigid rules and rapid early weight change can trigger relapse.[1]
Long term adherence is a genuine, well documented problem, not just anecdotal complaint. One review of ketogenic therapy for epilepsy found roughly two thirds of patients still following the diet at one year, but only about 38 percent still on it at three years, and separate weight loss program data showed similarly steep drop off well before the six month mark.[1]
Environmental, Economic, and Cultural Aspects
Because the diet centers on meat, dairy, and added fat while cutting grains and legumes almost entirely, its environmental footprint tends to run higher than more plant forward diets on the same environmental impact data, since the same large meta-analysis of food's environmental impact found red meat and dairy fat consistently among the highest emitting food categories per calorie or per gram of protein.[5]
Cost follows a similar pattern to the environmental one: fatty cuts of meat, cheese, butter, and nuts are generally pricier per calorie than the grains, legumes, and starchy vegetables the diet excludes, making sustained strict keto a comparatively expensive way to eat for many households.

- Breakfast: eggs scrambled in olive oil with spinach and feta cheese, plus half an avocado.
- Lunch: a grilled salmon fillet over mixed leafy greens with olive oil dressing and a handful of olives.
- Snack: a small portion of macadamia nuts or walnuts.
- Dinner: roasted chicken thigh with skin, sauteed broccoli and cauliflower in butter, and a spoon of pesto.
- Evening, if needed: a small serving of full fat cheese.
Who It's Best Suited For — and Who Should Use Caution
Best suited for children and adults with drug resistant epilepsy under specialist care, and, with medical supervision, adults with obesity, high triglycerides, or type 2 diabetes who have not responded well to other approaches.
Caution, and in some cases outright avoidance, applies to pregnant women, anyone with a history of an eating disorder, people with pancreatitis, liver failure, or certain inherited fat metabolism disorders, and anyone using SGLT2 inhibitor diabetes medications, given the specific risk of euglycemic ketoacidosis. Anyone with high cholesterol before starting should have it checked again a few months in, given how variable the LDL response can be.
